Breast Augmentation with Implants

Breast augmentation increases breast size and improves shape using implants placed behind the breast tissue or chest muscle. It's one of the most commonly performed plastic surgery procedures in New Zealand, with well-established safety and outcomes data spanning decades.

Round vs Anatomical

Implant Sizing

Sizing uses your chest width, tissue quality, and desired outcome — not cup size. Physical sizers are used at your pre-op appointment so you can visualise the result before committing.

Incision Location

Usually in the natural breast fold (inframammary) — discreet, heals well, and gives excellent access. Periareolar and axillary approaches are also available when appropriate.

Important Information

Risks, Complications & Informed Consent

Every surgical procedure carries risk. The vast majority of breast augmentation patients have no significant complications — but Dr Yaprak will discuss all of the following with you at consultation, and you will have ample time to ask questions before signing consent. This is not a procedure to rush into.

General surgical risks

  • Wound infection — uncommon, treated with antibiotics; rarely requires implant removal
  • Haematoma (bleeding) or seroma (fluid collection) around the implant — may require surgical drainage
  • Blood clots (DVT or pulmonary embolism) — risk minimised by early mobilisation
  • Anaesthetic risks — modern anaesthesia is safe; your anaesthetist will discuss these
  • Scarring — all incisions scar; scars fade significantly over 12–18 months but are never invisible
  • Wound healing problems — increased risk in smokers; stop smoking 6 weeks before surgery

Risks specific to breast augmentation

  • Capsular contracture — scar tissue tightening around the implant (see FAQ below)
  • Changes in nipple or breast sensation — usually temporary, can occasionally be permanent
  • Implant rippling or wrinkling visible on the skin
  • Implant displacement, rotation, or malposition
  • Asymmetry — differences in size, shape, or position between breasts
  • Implant rupture or silicone gel bleed
  • BIA-ALCL — rare lymphoma linked to textured implants only (Motiva and Mentor smooth implants are not associated)
  • Chest wall minor deformity from tissue expansion
  • Reports of connective tissue symptoms — the relationship to implants is suggested but not conclusively proven

Blood thinners & supplements: Tell Dr Yaprak about any prescription blood thinning medications. Also stop herbal supplements that affect blood flow — including St John's Wort, Ginkgo biloba, and garlic supplements — before surgery. Vitamin C is fine to continue.

Emotional & psychological considerations

Some patients feel emotionally "up and down" in the weeks following surgery — this is normal and usually passes. If feelings of depression or anxiety persist, please let Dr Yaprak's team know. Counselling may be recommended.

The decision to have breast augmentation should be yours alone, made without pressure from anyone else, and only when you feel genuinely well-informed. At least two consultations before surgery is standard — further consultations are always available.

Fat Transfer Breast Augmentation

Fat transfer augmentation — also called autologous fat grafting — uses your own fat harvested by liposuction, purified, and injected into the breasts. No implants, no foreign material.

It suits patients wanting a modest, natural increase of roughly one cup size, particularly those who also want body contouring in the donor area (typically abdomen, flanks, or thighs).

Fat transfer may suit you if:

  • You want a subtle, natural-looking size increase
  • You prefer no implants and no synthetic material
  • You have available donor fat elsewhere on your body
  • You'd also benefit from liposuction contouring
  • You want to avoid a potential replacement procedure in 10–15 years

Important: Fat transfer is not suitable for patients wanting a significant size increase, or those with very little available donor fat. Some fat reabsorption (typically 30–40%) occurs after transfer, which is factored into the surgical plan. Dr Yaprak will advise honestly whether this approach suits you at consultation.

Implant Removal & Replacement

Removing or Replacing Existing Implants

Many patients come to Dr Yaprak having had implants placed elsewhere — sometimes decades ago — and are now considering removal, replacement, or revision surgery. This is a growing area of practice.

Common reasons for removal or revision

  • No longer wanting implants — returning to natural shape (explantation)
  • Capsular contracture — hardening of scar tissue around the implant
  • Implant rupture or gel bleed
  • Implant malposition or visible rippling
  • Changes in shape, symmetry, or size over time
  • Breast Implant Illness (BII) symptoms
  • Older-generation implants reaching end of recommended lifespan

On Breast Implant Illness: BII is a term patients use to describe systemic symptoms — fatigue, brain fog, joint pain — that some women attribute to their implants. It is not a formally diagnosed condition, but Dr Yaprak takes these concerns seriously and will discuss the evidence openly with you.

Capsular contracture

The body naturally forms a scar tissue capsule around any implant. In some patients this capsule tightens and hardens — causing firmness, distortion, or discomfort. Treatment involves removing the implant and the capsule (capsulectomy). A new implant can be placed at the same time if desired.

Replacement & upgrade

Implants are not lifetime devices. Most manufacturers recommend reviewing them after 10–15 years, or sooner if a problem develops. Dr Yaprak can discuss modern implant options, sizing adjustments, and whether combining a lift (mastopexy) would improve the final result.

Are You a Good Candidate?

Every patient is assessed individually at consultation. These are the general indicators of suitability — but the most important step is always a conversation with Dr Yaprak.

  • Good general health with no active infection or untreated conditions
  • Non-smoker, or able to stop 6 weeks before and after surgery
  • Stable weight — significant changes after surgery affect the result

What to Expect — Step by Step

Consultations — at least two before surgery
Discuss goals, handle implant samples, take measurements. Dr Yaprak will advise what's achievable. Further consultations are always welcome — we want you to feel completely ready before proceeding.

Pre-operative preparation
Two online pre-op forms (hospital + anaesthetist). Stop smoking and nicotine products 6 weeks before surgery. Stop herbal supplements that affect blood flow. Rest the weekend before your surgery — overexertion close to the date can affect your recovery.

Get ready at home
Stock groceries in advance. Arrange help with chores and childcare for 1–2 weeks. Put essentials at easy-to-reach levels — you won't be lifting arms above your head. Have a front-opening button-up top for discharge day. Soft front-opening sports bras (Kmart, The Warehouse, Farmers) ready for 6 weeks post-op.

Surgery day at Braemar Hospital
Fast for 6 hours before admission (water OK up to 2 hours before). You'll meet the pre-op nurse, anaesthetist, and Dr Yaprak before surgery. 1–2 hours under general anaesthesia. Day stay — home the same evening. Your driver will need to stop at a pharmacy to collect your prescription on the way home.

First week home
Tape over your sutures — keep dry for 48 hours, then you can shower over the tapes (dry thoroughly with a hairdryer afterwards). Swelling and tightness are normal. Short gentle walks around the house are fine. Sleep propped up on extra pillows or in a recliner to reduce swelling. No lifting arms above your head, no heavy lifting, no driving for about 2 weeks.

1-week follow-up appointment
Wound check and tape replacement at our rooms. Scar taping continues for about 6 weeks. Most patients return to desk work around this time.

Weeks 2–6
Longer walks from week 2–3. No strenuous exercise or heavy lifting for 6 weeks. Avoid lifting children for 6 weeks. Wear your soft sports bra continuously for 6 weeks. Sleep on your back for 2–3 months for comfort while implants settle.

8 weeks — underwire bra fitting
Get fitted for a supportive underwire bra at around 8 weeks. Continue wearing supportive bras daily from this point. Further follow-up at 3 months and 1 year. All included at no extra cost.

3–6 months — final result
Implants soften and settle into their final position. Swelling fully resolves. Always let mammogram clinics know you have implants so they can adjust imaging technique accordingly.

What implant size will suit my body best?
Sizing is guided by your chest width, existing breast tissue, skin quality, and the result you want — not by cup size alone. Dr Yaprak uses physical measurements and sizers at your pre-operative appointment so you can see and feel different volumes before deciding. The goal is always a result proportionate to your frame.
Should the implant go above or below the muscle?
It depends on your anatomy and goals. Submuscular placement (under the muscle) gives a more natural appearance and makes future mammograms easier to read. Subglandular (over the muscle) is a shorter operation with quicker recovery. Dual plane — partially under the muscle — often gives the best of both. Dr Yaprak will advise what's right for you at consultation.
How long do breast implants last?
Modern cohesive gel implants like Motiva and Mentor don't have a fixed expiry — there's no need to routinely replace them after 10 years if everything is fine. That said, they are not lifetime devices. The general recommendation is to have them assessed after 10–15 years, or sooner if any changes occur. Annual ultrasound or MRI screening is recommended to check implant integrity. Motiva implants carry a lifetime manufacturer warranty.
Will augmentation affect breastfeeding or mammograms?
Most patients can still breastfeed after augmentation, though there is a small risk of impact on milk supply depending on incision placement. For mammograms, implants require additional views — most radiology practices are experienced with this. Dr Yaprak recommends a baseline mammogram and/or ultrasound before surgery, and continuing your regular breast screening afterwards.
What is capsular contracture and how common is it?
Capsular contracture is where scar tissue around the implant tightens and hardens, causing firmness or distortion. It occurs in a small percentage of patients. Risk is minimised by careful surgical technique and the use of modern implants like Motiva, which has a micro-texture surface specifically designed to reduce contracture rates. If it does develop, it can be treated with implant removal and capsulectomy.
Should I wait until after having children?
Pregnancy and breastfeeding can change the shape and size of the breasts, which may affect the appearance of augmented breasts. Waiting until after completing your family is a reasonable approach — but many patients proceed before having children and remain happy with their result. This is a personal decision and one worth discussing at your consultation.
